Is this policy documented anywhere? And where are these discussions supposed to take place?

#316 established the precedent, but there have been other requests that come in sometimes where people want to add a "Worldwide OSM Something" (see #663 (comment)) without involving the OSMF or CWG, and I reject these requests. I really don't want this index to be exposing users to spam, or misrepresenting the OSM project in any way.

And where are these discussions supposed to take place?

For the CWG -- we haven't had a chance to discuss -- it simply hasn't been a priority and we've had other things going on. We have not commented directly on community index inclusion before, but it is a fair suggestion for CWG to do so for global channels. We'd need to consider criteria for evaluating inclusion, and then assess specific requests. At the moment, we do not have a basis for recommending inclusion and don't have a timeline for consideration.
