Automatic heading called "Table of Contents" with an option to fold or generated directly into a callout.

Hi!! Love the plugin. Till now I had been using someone's CSS script wherein the table of contents is in the summary callout which is also foldable. The only problem was that it didn't update automatically, which is resolved by your extremely useful plugin! However, I don't see a heading of "Table of contents" or folding option, if that could be added, that would be great!!! I am adding a picture of what I'm trying to say:
