Escalation Name erased when in escaltion is edited

What went wrong?

What happened:

• When selecting edit on teh escalation chain to update the team, the escalation name is erased in the editor.

What did you expect to happen:
-When teh editor is open the name of the escalation chain should be prepopulated with the existing escalation chain name

How do we reproduce it?

Navigate to escalations
choose an escalation chain
select edit
notice the missing escalation chain name
