Add a text box field for name of doctor to patient enrollment

Problem

As of now, the person listed as the patients clinician is not their actual doctor. We want to be able to see the name of the patient's doctor written on the health summary, not the name of their research coordinator

Solution

Add a text field to the patient enrollment dashboard screen for the name of the patient's clinician. This string should then be the one that is tracked over to the health summary upon export.
